2011 Recreation / RecPlus Coaches Application

The 2011 Recreational Program Head Coaches application is now available for download. This form should be used to apply for a volunteer coaching position for both regular Recreation team and Recreation-Plus team head coaching positions.

An application to be an assistant coach is not required as the club permits the head coach of recreation teams to identify his/her selections for one assistant coach and one team manager. (Note: All team coaches and managers will still be required to participate in the Fusion's risk management / background check screening process).

Upon completion, please follow the application submission instructions found at the end of the application.

Background Check Information

All Fusion Soccer Club Coaches, Trainers and Board Members are required to participate in the Club's background check screening program. The program requires one visit to the screening data collection center to provide fingerprints and file paperwork. Fingerprints are processed through the State of California Department of Justice Criminal Database for clearance to participate in club activity.

The fingerprinting process need only be performed once. If you have coached previous seasons and have already been printed you are in the system and don't need to be printed in subsequent years.

If you have been printed through a background check for another group or organization you must still visit the screening data collection center specifically for Fusion SC. Each group's program operates individually with no cross sharing of data or information.

Fusion SC covers the cost of the screening. All you need to do is visit our screening service, Livermore Livescan, complete a form, and provide your fingerprints. Please be sure to have your valid identification such as driver's license or passport with you at the screening.
